- No tobacco use for previous 48 months

- Build within Preferred Plus NT Build table
- Cholesterol within limits of the following table or less (not treated)
- Age 20-39 220
- Age 40-54 230
- Age 55 and over 240
- HDL/cholesterol ratio 5.0 or less
- No family history of cardiovascular, cancer, diabetes or kidney disease deaths prior to age 60
- No history of drug or alcohol abuse
- No private aviation or ratable avocations/occupations
- No more than 2 moving (driving) violations in five years; no DUI/DWI within 5 years
- Preferred NT
- Available at face amounts of $100,000 and over
- No tobacco use for previous 24 months
- Blood pressure 150/90 or less with no current treatment
- Build within 20% of average
- Cholesterol of 240 or less not treated, HDL/cholesterol ratio 6.0 or less
- No family history of cardiovascular deaths prior to age 60
- No private aviation or ratable avocations/occupations
- No DWI and no more than two moving violations in past five years
- Non-Smoker
- Available at all face amounts
- No cigarette use for previous 24 months (other tobacco use is acceptable)
- Non-tobacco users who do not qualify for Preferred NT
- All applicants under age 20
- Standard
- Available for all face amounts
- Cigarette use in the last 12 months
